POSITION SUMMARY: Provides direct behavioral health care counseling and consultation services to AOT Communicare consumers. Responsible for coordinating the provision of services on behalf of SMI clients served by AOT team, in gaining access to needed services. Provides direct behavioral health care counseling and consultation services to AOT consumers.

QUALIFICATIONS: Master's degree in behavioral health field, board-licensed/credentialed or master's degree in behavioral health field, having completed all clinical course work, eligible to take a board-licensed exam, to be passed within two years of hire, along with having two years' experience working with the severely mentally ill population.

SKILL SETS: Organizational and adequate communication skills, process verbal and written instructions, computer proficiency, methodical approach to problem solving, work under stress and tight deadlines, interact effectively with the public and staff. Ability to handle stress effectively and make decisions accordingly.

Other demonstrated skills are problem analysis, resolution and customer service skills.

PHYSICAL AND ENVIORMENTAL DEMANDS: Frequent talking and active listening when communicating to individuals, families, guardians, and staff. Possible exposure to communicable diseases, possible exposure to blood borne pathogens. Visual acuity necessary for appraising the appearance of individuals and their environment. Conduct home visits with individuals which may cause exposure to risk of unsanitary conditions, abuse and violence. Standing and sitting, occasional stooping. Travel will be required.


  1. Make referrals to appropriate levels of care and additional services as needed.
  2. Consult with referral sources to coordinate and collaborate on treatment plans as needed.
  3. Exercises sound clinical and ethical judgment in treatment decisions and responsibility.
  4. Develops therapeutic, supportive relationships with severely mentally ill client population.
  5. Follows all applicable regulatory guidelines.
  6. Assists with the development and implementation of Therapeutic Rehabilitation activities.
  7. Participates in diagnostic and treatment planning meetings.
  8. Completes all documentation in a timely manner.
  9. Assists in the evaluation of program referrals.
  10. Facilitates voluntary hospital admissions when clinically indicated and within the guidelines of the referral hospital.
  11. Maintains professional conduct to facilitate coordination of all clinical and non-clinical staff.
  12. Attends supervision sessions as directed by supervisor
  13. Perform other duties as assigned.

  2. Attends mandatory in-services and other training/educational programs as required.
