POSITION SUMMARY: Provides services to participants within facilities that are approved to received ID/DD PASRR Specialized Services. Assist the participant with skills acquisition, maintenance of functional status, and implementation and assistance of specified goals and objectives as determined through the Person-Centered Plan. Services shall be provided in accordance with applicable Kentucky Statues and Regulations.
QUALIFICATIONS: Shall be at least eighteen (18) years of age or older and have a high school diploma or GED. Some areas may require valid Class C State of Kentucky driver's license and have current mandatory insurance. Shall meet all regulatory required employment processes (background checks, TB assessment, drug screen, etc.). Experience working with adults with Intellectual/Developmental Disabilities preferred.
SKILL SETS: Adequate communication skills, ability to process verbal and written instructions, ability to use a computer efficiently, dependable, interact effectively with the public, staff, and participants/their family members, able to adapt to deal with frequent change, delays or unexpected events, strong sense of commitment. Knowledge of MS- Office preferred.
PHYSICAL AND ENVIORMENTAL DEMANDS: Visual acuity necessary for appraising the appearance of the participant and the environment. Frequent talking and hearing when communicating with participants, families, guardians, and staff. May be some lifting when assisting with ambulation. May be some stooping, kneeling and bending involved when assisting. Possible exposure to bloodborne pathogens. Possible exposure to communicable diseases or hazardous materials. Services provided will occur at facilities or in the community.
JOB SPECIFIC FUNCTIONS:
- Responsible to assist participant accomplish skill acquisition, maintenance of functional status, and implementation and assistance of specified goals and objectives as determined through the Person-Centered Plan.
- Assists in integrating the participant in their community/activities as directed by their goals.
- Completes service notes and billing through EHR, as required by regulation/policy.
- Works with the team to identify progress and barriers to treatment.
- Meets regularly with supervisor to discuss concerns, problems, and progress.
- Adheres to the Person Centered Plan of the participant served as developed by the team (if applicable).
- Participates in agency Continuing Quality Improvement (CQI) activities.
- Completes PASRR billing for contracted services (weekly, monthly, or quarterly) and processes the application documentation/paperwork for EHR.
- Processes PASRR intake documents.
- Manages EHR profiles for PASRR participants, as needed.
- Sends applicable service documentation to facilities monthly.
- Completes PASRR record reviews.
- Performs other duties as assigned.
MANDATORY REQUIREMENTS:
- Adheres to Communicare, Inc. policies and procedures.
- Attends mandatory in-services and other training/educational programs as required.
